Student Health Records and Patient Privacy
Accessing Your Health Records
Student Health Services (SHS) permanently maintains patient records on their Northwestern Medicine MyChart account. You may request information from your health record by completing the Form to Request Copy of Medical Records (PDF). Please provide at least 7-10 business days for release of records. Only records generated from SHS will be released.
What’s Included in Your Health Record
Each visit to SHS generates a medical record that includes your symptoms, examinations, test results, medications, allergies, and care plan. These records are protected by state and federal confidentiality laws and will not be released without proper authorization unless required by law.
Sharing Records with Other Healthcare Providers
SHS can share your health information with another provider for continued care once a signed release of information is received.
How Records Are Used Internally
Authorized SHS administrative and billing staff may access health records to assist with filing insurance claims or answering records-related questions.
Appointment Reminders and Follow-Up Communication
During your first visit, you'll be asked to sign a release form specifying who may receive your health information. SHS staff may also contact you by phone or email about appointments, follow-ups, or test results as permitted.
Communication with Parents or Guardians
SHS does not contact the parents of adult students (18+) about medical visits without authorization. In emergencies or if the student is unable to make decisions, SHS may contact a parent or guardian. For minors (under 18), some information may be shared with parents. Privacy Around Student Employees
Student employees do not have access to health records and are informed of strict confidentiality guidelines. SHS staff do not discuss personal health information in the presence of student workers.
When Health Information May Be Shared Without Permission
SHS does not release health information to parents, professors, or administrators without written consent, except in emergency or life-threatening situations. Charges may be discussed with parents or insurers only if permission is granted.
Legally Required Disclosures
SHS may release information as required by law, such as:
- To public health authorities
- To law enforcement
- In response to a valid subpoena
- To national security or intelligence agencies
- To prevent a serious health or safety threat
